Your personal plan
Your plan is about your life
A personal plan is made with you. It should use the way of communicating that works for you.
- It says what is important to you.
- It says what you want to work towards.
- It says who will help with each action.
- It says when actions will be checked.
- It can change when your needs or wishes change.
You can have support
You can ask an advocate, family member or trusted person to support you in a planning meeting.
You should still be at the centre of the meeting and decisions.

Four types of support
Day Services
A day service can help you learn, meet people, enjoy activities and take part in your community.
Residential Services
A residential service supports you to live in a community home and get help with the things you need.
Respite Services
Respite is a planned short stay away from your usual home. It supports you and gives carers a break.
Clinical Supports
A specialist may help when an assessment identifies a need. Ask which supports are currently available.
