Your personal plan

Your plan is about your life

A personal plan is made with you. It should use the way of communicating that works for you.

  • It says what is important to you.
  • It says what you want to work towards.
  • It says who will help with each action.
  • It says when actions will be checked.
  • It can change when your needs or wishes change.

You can have support

You can ask an advocate, family member or trusted person to support you in a planning meeting.

You should still be at the centre of the meeting and decisions.

You can ask for information using plain language, larger print, pictures, an accessible digital document or support from a person who can explain it.

Formal Easy Read documents should use clear words and helpful images and should be tested with people who use Easy Read. Please tell us which document you need.
