Person-centred support
You are an active partner in your support
Person-centred support starts with the person’s voice. It looks at what is important to the person now and what they want for the future.
- You are supported to make choices and communicate decisions.
- Your strengths, interests and goals are recognised.
- Your support plan is reviewed when things change.
- Your family, advocate or trusted people can be involved when you want them to be.
- Information should be explained in a way you can understand.
Your personal plan
A personal plan records what matters to you, the goals you choose and the support that has been agreed.
A key worker may help you prepare for meetings, track actions and make sure your voice is heard.
Applying for a service
A clear route to the Admissions Co-ordinator
- Contact the Admissions Co-ordinator. You, a family member, advocate or professional can ask for information.
- Share information about your needs. This helps Prosper Fingal understand the support you are looking for.
- An assessment is considered. The organisation must be able to meet your needs safely and appropriately.
- HSE approval and funding are required. Access to a service depends on the relevant HSE process and available resources.
- You are told what happens next. Ask for the information in plain language or another format if that helps.
